The MBL expects all staff and every program participant/registrant visiting the campus—whether as a student, visiting scientist, course faculty, or course vendor—to adopt precautions to mitigate the risk of COVID-19 transmission. As such, all groups listed above will need to complete the MBL’s vaccination registry and COVID-19 Safety acknowledgment and attestation before arriving on campus. These items are part of the registration and decision reply-form and a pre-requisite for obtaining an MBL identification and door access card (MBL Card).
Following submission and approval of an MBL registration, you will be e-mailed a link to the MBL’s vaccination registry and COVID-19 safety acknowledgment and attestation. The e-mail will arrive two weeks prior to your arrival at the MBL. Instructions for completion and submission of the vaccination registry and attestation are included in the link sent to you. The attestation must be completed as an online form, for reference we have posted the PDF of the attestation.
