COVID-19 Vaccination Registry
In July, 2021 the MBL announced that all faculty, staff, and other members of the MBL community are required to confirm their COVID-19 vaccination status through a secure registry.
Registry information is sent to all MBL registrants prior to their arrival on campus and it must be completed to obtain building access.
After completing the registry, individual records will be accessible only to a restricted number of staff to confirm compliance with MBL requirements. If further action is required, the individual will be contacted by Human Resources. The information in the vaccination registry will be maintained in accordance with applicable laws and public health regulations and will not be included in personnel files.
Upon completion of the vaccination registry individuals will e-sign the MBL’s Acknowledgment and Attestation Regarding COVID-19. All individuals regardless of vaccination status must complete the registry form and sign the attestation.
